WellPath is not a consumer wellness app. Every patient journey begins with a clinician — a physician or nurse practitioner at a concierge or direct primary care (DPC) practice — who reviews intake data, assigns personalized goals, and monitors progress throughout. The clinician portal gives providers a complete, real-time view of each patient's behavioral data, lab results, and adherence patterns, enabling faster clinical decisions and more meaningful conversations at every visit.

Central to the entire experience is the WellPath Score — a single, dynamic number that reflects a patient's overall health engagement across all seven pillars: nutrition, sleep, movement, substances, mental health, core care, and advanced protocols.
The score is not a vanity metric. It is composed of individual pillar scores, each built from behavioral score components that map directly to the patient's active goals and tracked data. Every food logged, every night of sleep recorded, every workout completed feeds into a transparent scoring chain that patients and clinicians can drill into at any level.
The score updates in real time as data flows in — giving patients immediate feedback on their choices and giving clinicians an at-a-glance view of engagement and progress.
The Cycle Model: How Progress Compounds
Health is not a destination. WellPath structures improvement as a series of cycles, each building on the last.
Cycle 1: Getting Started
The first cycle follows a straightforward path:
- 1.Onboarding — The patient completes baselines, syncs Apple Health, and gets oriented to the app.
- 2.Goal Setting — The clinician reviews AI-generated recommendations, customizes targets, and pushes an approved plan to the patient.
- 3.Active Phase — The patient tracks daily behaviors, works toward goals, and receives coaching. The clinician monitors adherence and progress in real time.
- 4.Retrospective — At cycle end, a comprehensive retrospective is generated covering lab and biometric starting points, recommendations activated, adherence patterns, and measurable improvements.
Cycles 2+: Where the Compounding Begins
Every subsequent cycle starts with a checkpoint phase — a structured reassessment that generates a retrospective of the previous cycle. This retrospective provides a comprehensive overview: lab and biometric starting points versus endpoints, which recommendations were activated, adherence rates, and measurable improvements to labs, biometrics, and behaviors.
From there, the cycle continues:
- 1.Checkpoint — Baselines are revisited and pre-populated based on actual logged data, weighted toward the most recent weeks and months. The patient confirms or adjusts. A full retrospective of the prior cycle is generated.
- 2.Goal Setting — Now informed by not just current baselines and clinical data, but trends over time: what worked for this specific patient, what didn't, and — drawing from the system's growing understanding — what types of goals drive the best adherence and the most clinical change across all patients.
- 3.Active Phase — New goals build on previous progress. Targets may be more ambitious, more refined, or shifted to new focus areas based on retrospective insights.

What We Track
WellPath tracks three distinct categories of health data, each serving a different purpose in the clinical picture.
Labs (Biomarkers)
Periodic blood panels covering cardiovascular health, metabolic function, inflammation, hormones, kidney function, vitamins, iron status, thyroid function, and more — 59 blood biomarkers for female patients, 58 for male patients (progesterone is female-only), plus 4 biological age measures across 11 clinical categories. These are the clinical anchors: objective markers that reveal what behavioral data cannot.

Biometrics
Continuous and periodic physical measurements — heart rate, blood pressure, body composition, grip strength, VO2 max, and more. Many flow automatically through Apple Health integration; others require clinical equipment like DEXA scans or InBody assessments.
Behaviors
The richest layer of data. WellPath tracks daily behaviors across all seven health pillars — every meal logged, every workout completed, sleep patterns, hydration, supplement intake, and more. This behavioral data feeds directly into scoring, goal tracking, and the AI coaching engine.
Baselines are the entry point for behavioral data. Rather than subjecting patients to a long, boring survey, WellPath uses baselines as an interactive onboarding experience that introduces patients to the app's functionality while collecting meaningful data. Patients answer questions about their current habits — typical daily protein intake, usual bedtime, weekly exercise frequency, caffeine consumption patterns — in a format that is both trackable and directly translatable to real data points in the system.
Baselines are not static. In checkpoint phases of cycles 2 and beyond, they are revisited and pre-populated based on actual trend data from the patient's logged behavior, with weighted averaging that emphasizes the most recent weeks and months leading up to cycle end. This means baselines evolve to reflect reality, not memory.

The clinician portal is where clinical decisions happen. Each patient's page is organized into focused sections:

A complete view of every patient's data with the ability to enter clinical measurements — lab results, biometrics, and baseline observations — alongside a live view of what the patient is filling in on their end during onboarding.
Goal Setting
The most powerful part of the clinician experience. The AI assembles, personalizes, and suggests goal recommendations. Clinicians can then fully customize every aspect: frequency, specific targets, preferred times, ramping schedules that gradually increase difficulty, and more. Once satisfied, the clinician reviews the plan with the patient and pushes it live to the mobile app.
Active Goals
A live operational view of the patient's current plan. Clinicians see real-time adherence data for every active goal, can schedule and activate new goals mid-cycle, pause goals that need adjustment, and end goals that are no longer appropriate. This is where day-to-day clinical oversight happens.

A live view of everything the patient has entered in the app and everything synced from Apple Health — behavioral scores, lab trends, biometric measurements, food logs, sleep data, and more — all presented together for clinical context.
Education
Built on the belief that building knowledge in a topic area improves adherence. This section shows the patient's assigned articles and allows clinicians to assign additional educational content to specific patients or their entire panel.
