Reassign or Change a Patient's Clinician

Hand a patient off to another clinician, or return them to the unassigned pool.

Every patient in your practice has an assigned clinician — the person responsible for their care. When that needs to change, you do it from Settings → Patient Setup, in the Actions column on the patient’s row. The action you want is Change clinician.

(There’s a separate Move to Soloaction right next to it that sounds similar but does something different — see below so you pick the right one.)

Change clinician

Select Change clinicianon the patient’s row and choose another clinician in your practice. The patient keeps everything— their goals, scores, baselines, logged history, and their sign-in — only the assigned clinician changes. You’ll see a confirmation that the patient has been reassigned to the new clinician, and they move onto that clinician’s panel.

Use this whenever care is handed off: a clinician leaves, a patient switches providers, or you’re balancing panels across the team.

Return to the unassigned pool

In the same Change clinician dialog you can choose Unassignedinstead of a specific clinician. The patient stays in your practice with all of their data intact, but has no assigned clinician until you pick one. This is handy when a clinician is leaving before you’ve decided who takes over — park the patient in the unassigned pool now, assign them for real later.

Change clinician vs. Move to Solo

These sit next to each other and are easy to mix up. They are not the same:

  • Change clinician keeps the patient in your practice. You’re just moving them from one clinician to another (or to unassigned). You stay in control of their care.
  • Move to Solo takes the patient out of your practice entirelyand converts them into a self-managed Solo account — they run WellPath on their own, with no clinician oversight. It’s a bigger, one-way change, covered in its own guide.

If the patient is staying with your practice, you want Change clinician.

Who can reassign

You can change a patient’s clinician if you’re the patient’s current clinician, a practice admin, or a platform admin. Regular team members without one of those roles won’t see the action, so handoffs always run through someone accountable for the patient.
